form can be printed, filled out and mailed or faxed to the Parish Office as indicated at the bottom of each form.

Adult Faith Formation

This form is for adults who believe they are ready to receive the Sacrament of Baptism, Holy Eucharist or Confirmation.

Catechists and Volunteers

Catechists and Volunteers Registration Form

This form is for those who wish to volunteer as a Catechist or other Assistant during the Faith Development year. Form can be mailed, hand-delivered, or placed in the collection basket during mass.

For families who wish to instruct their child at home. Form can be mailed, hand-delivered, or placed in the collection basket during mass.
